Pre-Screen Questionnaire

This questionnaire is a guide to help you determine whether you are eligible to apply for the Down Payment Assistance/ Closing Cost Assistance Program. Please note that eligibility based on this form alone does not guarantee approval.

Have you been employed for the last 90 days? *
Do you have the required minimum of $1,000 to contribute toward your purchase? *
Do you have a credit score of at least 620? *
Do you have a contract to purchase a home at the time of application? *
If your answer is no to this question, you may still be eligible to apply if you have a purchase contract to build within 60 days from this application date or case by case. Please contact us to help determine your eligibility.

If your answer is yes to questions 1-5 you are ready to proceed to the next step. After you submit this form, we will contact you within 10 business days regarding next steps.

Please submit your pre-approval for mortgage letter.
